Summary

Sixty-nine elderly patients over 65 years oldwith ruptured aneurysms had a significantly worse outcome than 192 younger patients with respect to overall management results and operative results (p<0.01). The results of an early operation were better in the elderly patients with grade I–III than in those receiving a delayed operation. Concomitant diseases significantly influenced the results in elderly patients (p<0.05). It is possible that an early operation results in the better outcome in elderly patients by decreasing the incidence of delayed ischemic complications and preventing rebleeding.

In patients with sSAH, increasing age is associated with a worse clinical condition at admission as well as a higher amount of blood on the initial CT.33,34 Moreover, the incidence of severe complications increases significantly with advancing age.34 Consequently, older patients have a significantly increased risk of poor outcome.34-36 However, despite these mentioned conditions, there is substantial evidence that older patients show a lower incidence and intensity of symptomatic and angiographic CVS.37-39

However, other studies6,19,25 have indicated that the most common site for aneurysms in elderly patients is the anterior cerebral artery. Several other studies have shown that elderly people are more likely to have aneurysms in the vertebrobasilar artery and the Acom, rather than the ICA.26 These studies may have reported discrepant findings because they did not all examine patients who were the same age.

Our study showed an increased prevalence of smoking in the SAH population under 35 years, compared to the general population. Sixty-two percent of our patients were active smokers versus 20.8% of the United States adult population [13]. The population-based study conducted by Woo et al. [11] also produced similar results with a smoking prevalence of 62.6% among acute subarachnoid patients.
